Trained aides regularly visit patients’ homes and provide assistance with bathing, grooming, and other care that assures the personal hygiene and safety of the individual. Aides may also prepare meals, change linens, help with laundry, clean the home and shopping. Most patients receive personal care under the Medicaid program.

Medicaid eligibility:
1. Care prescribed by physician.
2. Current Medicaid card.
3. Require assistance with at least two (2) activities of daily living.
4. Age – No minimum or maximum

Private eligibility:
1. Care prescribe by physician.
2. Age – No minimum or maximum

Non-Medicaid/State Funded eligibility:
1. Care prescribed by physician.
2. Meet income guidelines.
3. Require assistance with at least two (2) activities of daily living.
4. Age 60+
