TARGETED CASE MANAGEMENT provides assistance to Medicaid recipients ages 21 to 59 and 60+ who are disabled and need assistance in gaining access to needed medical, social, educational and other services provided under Arkansas Medical State Plan and other community services.

Eligibility:
1. No longer required, DHS RN approves TCM services under ARChoices waiver
2. Current Medicaid card.
3. Have no available and reliable supports to provide assistance.
4. Ages 21 to 59 and 60+

SENIOR CASE MANAGEMENT is a private pay case management program to assist recipients and/ or family/friends with information and coordination of services.

Eligibility:
1. Approved for service by Area Agency on Aging nurse.
2. State funded.
3. Age 60+

CLIENT REPRESENTATION provides assistance to non-Medicaid recipients in gaining access to needed medical, social educational and other services available in the community.
