We offer a variety of services to help our seniors age gracefully. Hospice and Skilled Care require specialized skills to help your aging loved one in their hour of need. We know how difficult this can be on both your loved one and your family. That’s why we’re here to help.
Hospice
Hospice is short-term assistance for terminally ill patients and families that is usually conducted at home by a licensed nurse, nurse aides, volunteers, social workers and/or clergy members. This assistance allows the terminally ill to remain in their own home and remain as comfortable as possible.
Hospice also works to educate the family on the terminally ill process. Bereavement Care starts upon admission and continues for family members after the passing of their loved one.
Medicare Eligibility
- Care prescribed by physician.
- Diagnosed as terminally ill with six (6) months or less to live.
- Not receiving curative treatment.
- Primary caregiver available 24 hours/day.
- Committed to home care.
- Age – No minimum or maximum
Private Insurance Eligibility
- Care prescribed by physician.
- Approval from insurance carrier.
- Age – No minimum or maximum
Private Pay Eligibility
- Care prescribed by physician.
- Age – No minimum or maximum
Skilled Care
Skilled nursing care refers to a patient’s need for medical care or treatment that can only be performed by licensed nurses. This type of care can be offered in the comfort of your own home by a certified nurse.
Skilled Care can offer a wide range of services and medical care:
- Physical therapy
- Speech-language pathology
- Wound care
- Intravenous (IV) therapy
- Administering and monitoring medication
- Injections and monitoring of vital signs and medical equipment
